It wasn't merely a matter of "inflammation." The real issue was

He stared at me, face etched with disbelief. “Does she… need surgery?”
“Yes. We need to move her to the ICU immediately and prepare for emergency
surgery.” His lips trembled as he struggled to speak. “Just a while ago… they
said it might just be an upset stomach… Doctor, my wife… she’ll live, won't
she?”
In that moment, I fell silent. Doctors often face moments where they must
prioritize the truth over false hope. “This is a highly fatal disease with a
mortality rate exceeding 50% if diagnosis is delayed,” I replied cautiously.
“But fortunately, we caught it now. We still have time to fight. The most important
factor is how quickly we proceed with treatment from here.” The husband lowered
his head, speechless. He just sat there, slowly crushing the straps of the bag
in his hands.

I returned immediately to the station. Amidst the frantic bustle of the
ER, residents and the surgical resident on duty began to gather. Ms. Park’s
coronal CT scan was pulled up on the monitor. I zoomed in.

"diffusivity." The fascia is a thin membrane designed to protect
organs, but paradoxically, once infected, it becomes a high-speed highway for
bacteria. When anaerobic bacteria are involved, tissue necrosis progresses even
faster.
Suddenly, a resident spoke up urgently. “Professor, the blood work is
back. Her lactate level is 4.5.” The air in the room grew heavy. Lactate

hypoxia.

now.” The resident scribbled quickly. “Starting carbapenems.” “Add anaerobic
coverage as well. We need to cover everything—Gram-positive, Gram-negative, and
anaerobes.”
The preparations for emergency surgery moved in tandem: securing an OR,
prepping an ICU bed, ordering blood, and paging the anesthesiologist. The ER
transformed into a literal battlefield within seconds. Yet, a nagging anxiety
remained in the corner of my mind: “Did I miss anything?”
Traces of the Invisible Infiltrator
I went back up to the reading room. The room was dark, save for the dim
glow of the monitors. My coffee had gone cold, but I didn't have a second to
touch it. I re-examined the images from the beginning. Axial. Coronal.
